You can create surveys, feedback forms, and even self-grading quizzes with Google Forms. Perhaps you want respondents to rank a list of items. Here’s how you can do that. How to Make a Google Forms Ranking Question?

Use the Multiple Choice Grid Question Type
You can list as many items as you wish in the rows and use the columns for ranking in Google Forms’ Multiple Choice Grid question type.
The form itself can then limit answers to one per column. It ensures that no two items have the same ranking. Each row can also require answers for each row.
You might ask respondents to rate on a scale of one to five which product attributes are most important to them. Alternatively, you can ask respondents to rank the likelihood of participating in each activity.
Multiple Choice Grids also allow you to shuffle the rows. This tool is useful if you want the question to appear different every time someone opens the form.
Create the Ranking Question in Google Forms
Open Google Forms and sign in if necessary. For our example, respondents will rank product attributes from least to most important.
Add a new question by clicking the plus sign in the floating toolbar on the right. On the right side of the question that appears, choose “Multiple Choice Grid” from the drop-down box.

Each ranking should be added as a column. You’ll need at least the same number of columns and rows if you plan to limit respondents to one answer per column (described below).
Add the columns
By placing your cursor over a row or column and dragging the grid icon on the left, you can rearrange them. You can remove a row or column by clicking on its X.

Click the three dots in the bottom right corner and select “Shuffle Row Order” to change the order of rows with each open form.
Select Shuffle Row Order.
Turn on the toggle at the bottom for Require a Response in Every Row to require an answer for each row.

Respondents will see an error message if they submit the form without responding to each row.

By clicking the three dots on the bottom right of each question, you can limit the answers to one for each column. Just select this option.
